2022-23 Program Ads
Thanks for supporting Flower Mound Theatre

We've gone paperless! All ad forms are electronic this year.
Each advanced theatre student is required to sell one ad.

Step 1 -- Fill out the form online
You can ask the person buying the ad to fill out and submit the form.
The first "email" question is for the student email,
so the student will get a confirmation the form has been submitted.
Step 2 -- Submit Payment via Venmo
Venmo details on form. You'll include payment date on form.
If you are not able to pay via Venmo, we will accept checks.
Please have the student bring the check to school and put in the Booster Club Dropbox.
Or mail the check to:
FMHS Theatre Booster Club, P.O. Box 270958, Flower Mound, TX 75027-0958.
Payment must be received by August 20, 2022.
Step 3 -- Submit ad content
Please submit your ad content (either camera ready art or elements to include in an ad we create for you) directly through the Google form. Or email the ad/ad content to firstname.lastname@example.org
Ad content must be received by August 20, 2022, for your ad to be included in the program.
